What can you expect in most cases during a massage session? For daily stress, discomfort and maintenance, massage is highly recommended in all forms. Before starting your own massage business, you already need to have massage therapy experience, so I will not be covering that aspect. After all, you know you are ready to start your massage therapy business when your customers are coming back for more, and when the money you earned from your side-gig as a masseur is no longer considered 'pocket money'. Or maybe you have worked at a spa or for somebody else and now you are ready to take the plunge and go on your own. You can do this, and here are some tips to get you started.

When you start a new business, any kind of business, the first step is making a business plan. What type of massage will you be offering to your clients? Most therapists learn 2-3 different styles and specialize in them, with focus on one particular style. It is good to either have a certification from a massage school, not only to hang if from your wall, but to increase your credibility with your clients and really know that you can do it.

The next step is actually becoming a business. You need to register a company name and the business type. Usually your local Chamber of Commerce will be able to help you with this step. The possibilities here are endless as long as you attract customers and make them return.

Massage therapy is a treatment option which can reduce pain and discomfort in the soft tissue surrounding a fracture injury, and it can also assist in reducing edema and muscle tension of both the affected and compensating structures. When a massage therapy treatment plan is carried out correctly, once the cast is removed, a client can regain full function sooner as the health and mobility of compensating muscles has been addressed.

Initially, your massage therapist will only work above the cast, and will take care to not disturb the healing bone. Eventually, when the cast is removed, massage therapy will continue to restore the health of the tissue by increasing circulation, restoring function and strength, and reducing any scar tissue.

Considerations for treatment

It is highly recommended that your physician provide a written recommendation for massage, and your therapist probably won't agree to treat you until your doctor has given the green light.

In all likelihood, you will also be seeing a physical (physio) therapist. It's a good idea to provide written permission for your massage therapist to collaborate with your other health care professionals to ensure the best possible care and treatment outcome.

Before your massage therapist begins to treat you, he or she will want to verify the following information:


  • your physician has approved massage therapy

  • are you on any type of medication? (anti-inflammatory, analgesic, anti-coagulant, antibiotics).

  • what is your general health - for example, consider age, fitness prior to injury, nutrition (your MT may refer you to a nutritionist, as proper nutrition impacts the healing process), lifestyle (i.e. do you smoke?)

  • are there any open wounds which must be addressed? (i.e. with a compound fracture)

  • Will treating the affected limb with massage therapy stress and disrupt the healing of bone, muscle, skin or nerve tissue?

  • is there any neurological or vascular impairment in the area which prevents the use of certain massage techniques or modalities?

  • do you have any plates, wires or pins? If so, hydrotherapy over the site will be contraindicated.

  • the most important consideration is to avoid stressing and interfering with healing tissue. Any pressure or traction applied to the affected bone is contraindicated.

  • have other types of injuries been sustained, such as sprains or contusions? (in all likelihood, there will be other injuries)

  • muscle atrophy from disuse will be present when the cast is removed. Provided that the injury is not an avulsion fracture, the development of atrophy can be reduced by the use of isometric contractions at the appropriate time with cast on. If an avulsion fracture is present, avoid isometric contractions, as this may disrupt the healing bone.

  • tissue under the cast will be fragile when the cast is removed, so extra care must be taken when massaging this tissue

As treatment progresses and the injury heals, issues such as tissue atrophy, altered biomechanics and altered proprioception must be taken into account. For this reason, there are different considerations for treatment with cast on and cast off.

Types of fractures

There are several basic types of fractures:

1. Simple - there is no broken skin, and the break is clean. Also known as a "closed" fracture.

2. Compound - the broken bone damages surrounding tissue and skin by piercing it. There is a greater risk of infection in this type of break due to breaking of the skin and protrusion of the injured bone.

3. Comminuted - a fracture that is in several pieces - common among a population with more brittle bones (i.e. elderly)

4. Compression - a fracture where the bone is crushed

5. Depression - a fracture where the bone is crushed and pressed inward

6. Impacted - bony surfaces are forced into each other (impacted)

7. Spiral - an break with ragged edges that result from twisting

8. Greenstick - an incomplete break that is common in children, as bones are more flexible

9. Stress fracture - a.k.a. hairline fracture, is a tiny crack in the bone and may not be visible with an x-ray

10. Avulsion - this occurs when a ligament or tendon pulls away a chunk of bone to which it is attached.

Fractures and breaks are repaired by "reduction" - when the bony ends are placed back together. A closed reduction is done manually by a physician, whereas an open reduction involves surgery, and possibly the placement of pins or wires to hold the bony surfaces together.

Finding the best massage therapist for you

It goes without saying that an injury as serious as a fracture or break will require treatment from a therapist who really knows his or her stuff and understands the healing process. Personally, I would ask my physical therapist if he or she could refer me to a qualified and experienced massage therapist, and then ensure that I give them both permission to communicate with each other so that my treatment plan would address all of my needs and concerns.

Have you ever started a new workout routine and felt sore in the same place all the time? How did this hold you back from advancing and making the most of your weight training? Most people experience this feeling of muscle spasm at some point in their strength training routine, and it holds them back from advancing because it is not common lactic acid soreness, but a serious muscular imbalance that can lead to injury. Balancing muscle groups increases strength and flexibility; for example, the pecs are commonly short and hyper-tonic which protracts the scapulae causing neck and upper back pain.

We all have our areas in which one muscle group is elongated, and another one is shortened. For example, look in the mirror and see if one shoulder hangs lower than the other or look at yourself sideways and see if your shoulders sit in front of the body's coronal plane. Imbalances are very common in the lower body too. Lay on your back and see if one foot naturally splays out more than the other. In addition, one leg usually is slightly shorter than the other.

Many common factors cause muscular imbalances; for example, repetitive motions at work, incorrect postural habits, scoliosis, old injuries, and emotional holding patterns. A good way to get strong is by lifting weights to work both the agonist and antagonist muscle groups for a given area. Let's use the pectorals (chest) group and the latissimus dorsi (back) group as our example. While doing pull-downs, some people might feel a spasm as if one side of the lats were shorter than the other, like the left side goes down before the right side and is tighter. This could cause a shortening of the left side of the latissimus dorsi, and might even cause the trunk to twist slightly left, with the right pec and anterior deltoid being shortened. This can cause pain that prevents a person from doing this exercise, even though it is good under normal circumstances.

One effective treatment is to have this client lay on his right side, and for the massage therapist to lengthen the left lat and serratus anterior. The therapist also has to release tight muscle bands in the deltoid and the rotator cuff, as well as under the shoulder blade. Then, with the client supine, the massage therapist works to release tight pectorals and the anterior deltoid of the right side. In this example, the SCM and trapezium would also need attention because the entire shoulder girdle and neck area will be affected and in need of relief.

My name is Manu Kalia. I am a physical therapist. In this article I want to share a few simple tips that I teach my patients to help them manage their low back problems. Eighty percent of the population is affected by low back issues, so I think this is a really important topic.

Every low back pain is different or unique in its own way. Each patient needs to figure out what aggravates the symptoms and what alleviates or relieves the pain. That's very important to understand. Also, the body will heal itself but you have to give it the right environment to heal. Medications may provide temporary relief but they do not necessarily address the underlying issue.

The first thing to find out is the cause, that which aggravates the problem or increases the problem, and then to remove that cause. I always tell my patients that if I hit myself with a hammer and I continue doing so, I do not give the injured tissue time to heal? I keep aggravating it, keep injuring the tissue and do not allow enough time to heal. Same way most of us keep pushing it even when we are hurt or injured. So it's very important to figure out the activities that are causing you more pain, and avoid repeating them.

The first concept to think about is whether your back is sensitive to load bearing activities (like standing, sitting, bending, walking, etc.); and if it is load sensitive you must avoid these positions. So always ask yourself, does this increase my problem? If so, I have to unload the spine to get relief from back pain.

Unloading can be done by lying down, whether you are lying on your stomach, on your side or on your back -- any of those positions that are comfortable.

In physical therapy I often use manual or mechanical traction for my patients. This is similar to using an inversion table to unload and relieve the compression on the spine - basically removing the pressure off of the joints, the discs, the nerves and the local structures.

Second important point to remember is whether the problem is made worst by static positions or with movement? If static positions like sitting or standing in one position increase the pain then it's best to get out of those positions and move frequently.

When movement makes the pain worst, it's best to avoid excessive movement or excessive activity. Whether it's housework, yard work, exercise or other physical activities, you have to moderate the activity level. This will give the injured tissue time to rest and heal.

Another important point to consider is whether the pain is made worst by bending or arching the spine. Positions that cause the spine to round forward like sitting, bending, etc. will cause more pain. On the other hand if the pain is made worst by arching the back as in standing upright, arching backwards or lying on the stomach, then these positions should be avoided.

It is very important to understand which of these positions or activities, cause you more pain. Once you identify the painful positions, avoid those positions and do more of the opposite to allow the spine to heal.

Now that you have decided to try massage therapy; you need to make an informed decision as to what type of establishment and who would best suit your needs.

First, you must decide what type of massage therapy office you want. Will you be comfortable going to a "spa" that provides a lot of frills and extra's. These establishments are alright, but you will probably have additional charges for the extras; or the extras will be "built in" to the price of the massage. This is nice, and preferred by some people because they like to be pampered. The other side of the coin is a professional massage therapy office. These establishments are usually simple in design, clean, and comfortable. Anyone will feel at ease when entering one of these establishments. A benefit of a professional massage therapy office is the services are priced lower, because the office has a lower overhead compared to Day Spas.

The massage establishment has been chosen, now you have to choose a therapist. The first thing to consider is will you be more comfortable with a male or female therapist. This is strictly a personal preference, but it can limit your choice of massage establishments. Not all places will have both male and female therapists available.

After deciding on the preferred gender of your therapist; now it's time to do some research. First, ask friends, family, and co-workers if they can recommend a good therapist. These personal referrals are some of your best starting points; but keep in mind, massage is very personal and what one person likes another may not like.

Most states require licensure for massage therapists. Check your states laws, if it is required, then make sure the therapist you choose is licensed. You can also easily check if the therapist has had any complaints lodged against them through the state massage board.

Meet your therapist. This can be done in person (the preferred method) or you can talk to him/her over the phone. Ask as many questions as you feel necessary. Asking how long they have been practicing is ok, but a better question is to ask how many massages the therapist has performed. Now don't expect an exact number, but a ballpark figure will suffice. The reason for asking this question, in this manner, is some therapists do more massages than other therapists. A therapist who has only been in practice for a year may have more actual hands on experience than someone who has been in practice for 2 or 3 years.

Some additional questions to consider asking:

1. What certifications do you hold?
2. What type of draping techniques do you practice?
3. What massage modalities do you use?
4. What kind of lotion/ oil do you use?
5. How long does the massage session last?
6. What is the cost of a session?
7. Do you offer package deals or specials?
8. What type of payment is accepted? (cash, check, credit/ debit card)
9. Do you have references?

Professional massage therapy is an intimate form of holistic medicine. There will be some things can not be asked; they have to be felt intuitively. Do you feel a connection with the therapist you have chosen? Does the therapist exhibit confidence in himself/ herself? Do you feel comfortable with the therapist? It doesn't matter what credentials the therapist has, if you don't feel comfortable, you will not be able to relax during your session. The client/ therapist relationship is similar to patient/ doctor relationships. They are built on trust.

After your massage therapy session you should evaluate your experience. Did the therapist treat you as you expected? Did you feel comfortable during the massage? Was it a positive experience? Would you recommend the therapist? If you answered yes to these questions, you should relay your feelings to your therapist. This can be done by leaving a gratuity, booking another appointment, or leaving a written testimonial/ thank you note.

Choosing a massage therapist should not be taken lightly. But it doesn't have to be difficult. Trust your intuition above all else. Communication between you and your massage therapist should be open and free. A good professional massage therapist will meet your needs, make you feel comfortable, and treat you with respect.
